You are a state, local, or federal law enforcement officer or public official whose position exposes you to a threat of death or serious bodily harm. b. You are a victim of identity theft. c. You are at risk of physical harm. d. You have evidence that the record is incorrect or expunged. 
2. Submit the following information: 

a) A written explanation for the opt-out request. Identify the specific location of Your Data on our Website and, if your personal Data is publicly available, identify one of the four conditions enabling you to opt-out. Describe why such publicly available personal Data is inaccurate and harmful.

b) Provide a copy of your current driver’s license or state identification to verify your identity.

3. All requests must include: 

• Full name and date of birth 
• Any aliases you use 
• Your current address 
• Previous addresses 
• Phone number 

• Email address 

4. Provide specific, complete details of the records you want to be removed. 

5. Include a printout of the records you wish to have suppressed.
